Experience & Requirements

  • Graduate of an accredited School of Nursing

  • Licensure as a Registered Nurse in the state of employment.

  • Must be certified in wound care by a national organization such as the American Board of Wound Management or the National Alliance of Wound Care and Ostomy

  • Current hands-on BLS certification per Corizon Health Credentialing Policy

  • Previous Wound Care experience preferred

  • Possesses an active CPR certification.

  • Remains knowledgeable about specific state laws and regulations governing practice.

  • Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.

A background check, security clearance and drug screening are required for this role.

Below is a list of your responsibilities as a Certified Wound Care RN with YesCare:

  • Performs, in collaboration with the Wound Care Physician, weekly Infirmary Rounds. All newly admitted Infirmary patients will be screened and their relative risk for skin breakdown reported according to their Braden Score. Assessment of all infirmary patients that are bed-bound and/or wheelchair bound will be reviewed. Documentation of all encounters in patient's Medical Record in SOAP format, problem oriented and corresponds to the therapeutic order. Utilizes a systematic approach to meet the healthcare needs of each individual patient within their respective region.

  • Facilitates Outpatient Clinic Rounds at least monthly. Patients with existing wounds, HgbA1C greater than 9.0, those with suspected peripheral arterial disease, prior amputation and those dependent on hemodialysis will be screened and followed every 3 months or sooner if problems arise or condition warrants. Consults generated by site Physicians for assessment will be scheduled and seen as part of the Outpatient Clinic Rounds.

  • Assists in implementation of a wound care protocol within their respective SDA, as well as a routine follow up/risk assessments plan. Also assists in stratification of more difficult cases to the Wound Care team at the site level at regular intervals. There will be plans for specific protocols including care of lymphedema, venous leg ulceration, diabetic foot ulceration, decubitus ulcers and peripheral vascular disease. There will also be plans for a wound care formulary whereas various supplies will be readily available and will fall within written protocols. (ex: multilayer compression wraps for venous leg ulcers)

  • Responsible for developing a plan of care/action based on clinical findings and modify plan of care as needed.

  • Ensures accurate and timely data collection within their respective SDA and will assist in generating frequent reports to the state with regards to patient’s wound care progress. These will including frequent measurements, culture data/antibiotic therapy and resistance, as well as the patient’s response to therapy. Clinic lists will also be readily available to report the number of patients seen in the outpatient setting.

  • Demonstrates proper technique for cardiopulmonary resuscitation and related drug therapy.

  • Supports standards of medical care through adherence to existing policies and procedures. Serves as a resource to other professional or non-professional personnel providing instructions as needed.

  • Assists in other nursing duties as assigned and will report directly to the Statewide DON.
